靈寳大法司。

當司今月某日，伏爲黄籙大齋主某人，滿散无上黄籙大齋，祗設清醮三百六十分位，上謝洪恩。所有進拜青詞五函，及功德䟽一函，各已恭對太上御前宣讀訖，并繳進狀二函，並重封印全。須至關發者。

右關某日當直功曹使者云云。同齋詞關式。如散醮夜深，關兩日功曹發式具六卷詞䟽門。
