某道某府某縣某郷某坊巷，居住奉大道弟子姓某。
右某伏爲本家某人，年紀月日時生。於甚年月日得患甚病，醫藥無効。切慮邪祟纏逐在身，枉傷性命。或夢中交徃男女神鬼，或因卜出，子細於狀内稱說。謹具狀投。
靈寳淨明院外壇。伏望法慈，救治施行。謹狀。
