水簡
如山簡式，謁靈泉、海瀆等處投之
告文
今謹有略節意於某處修建黄籙大齋幾晝夜，以今滿散，投簡水府，願神願仙。九府水帝，十二河源，江河淮濟，溟泠大神，乞削罪簿，上名九天。謹詣水府，金龍驛傳。
年歲次 月 日，具位臣姓某於處告下。
