北斗五雷主，黑暗鳴天鼓。風伯掃瘟黄，電母除病苦。霹靂伐妖精，雨師行法雨。灌漑入三焦，清涼通臟腑。重處即時輕，損處即時補，熱處即時涼，積處即時去。危處即時安，痛處即時住。急急如神霄眞王大帝勑。

默念祕呪勑符天醫許總管仗劔部五雷三醫諸將，隨符乗風雲，徃患家治病，立候報應。
