右臣先據入意同前。謹再録狀，上奏。
東極青宫太乙救苦天尊玉陛下，恭望。
道慈，允臣所奏，特敕飛天神人下九龍符命，遍詣十方三界地獄幽關云寫皆同昊天狀式。
