上淸眞人潘尊體厷，建弘道壇於嵩之陽。句曲外史基石儀道，廼復古初。廼奉聖神，永作憲矩。右三銘，泰定甲子歲玉虛子撰，上淸宗師劉大彬作篆鎮壇。茅山志：大洞法師倪天奎拹誠購石。
